As of 12/7/06, PMI released a new application for insurance. The new application is a combination of the Standard Application and PDQSM (delegated) Transmittal forms.
Select the state in which your property is located and enter your information on the document formatted in Adobe Acrobat 6.0 or later. Please remember to enter your Customer Name and Branch # on the application.
Complete the form online, print, and fax to a PMI Regional Operations Center. The fax number is right on the form.
